Pre-Appointment Tools

How to Make an Appointment


All patients seen in the clinic by the Movement Disorder Neurologists, Geriatric Psychiatrist, and Neuropsychologist must be referred by a physician or nurse practitioner with referring privileges.

The referring physician will need to fax a consultation letter or the MDC "Request for Services" to 204-940-8414. Our waiting list is currently 8 months to see a new patient. It is up to your physician to decide how to manage your health issues until you can see us. Your physician can call us anytime for advice.

Other neurologists in the community can also be of great assistance in the care of Movement Disorders.

Neurologists can access some of the other clinic services (other than a Movement Neurology Consultant) directly.

The clinic resource nurse can be contacted for information as needed. If you would like to see this nurse in the clinic though, an appointment would need to be arranged in advance.
