We offer a limited number of forms and policies online for the convenience of those who have already been referred to us. All forms require Adobe Acrobat to view. If you have questions about any of these forms, please call us at 831-3420.

Intake Package: To be completed by those who have an intake appointment. This is not a referral form. To get a referral, contact the appropriate federal referral bodies to see if you are eligible. Please note that if you have an intake appointment scheduled, this form will also be mailed to you.

Confidentiality Policy: As a public clinic, we follow a strict confidentiality policy that conforms with provincial legislation and is respectful of patients' privacy.

Code of Conduct: We work to ensure a respectful environment for everyone at the clinic.
